Pre-Participation Examination (PPE)
Every athlete competing at Taft College must complete an annual physical screening before they will be allowed to practice. The physical must be performed by a licensed physician.
Physicals will not be accepted from; Chiropractors, Nurses, Dentists, Physical Therapists, Massage Therapists, or anyone else who is not a licensed physician, or working directly under a licensed physician.

Physicals must be performed on our standard form which can be downloaded here. PPE forms are also available from the Athletic Department secretary, or the Athletic Trainer. Each form must be clearly filled out, signed by the licensed physician and include the physicians name, address and phone number and date of exam. The form can be delivered to the Taft College Athletic Department through the USPS or delivered by hand at the Al Baldock Cougar Sports Center. This must be completed and accepted prior to an athlete participating in any practice.

Insurance & Medical Information
If you will not be at least 18 years of age by the start of practice time for Fall sports athletes, or the beginning of school for all other athletes, you must have your parents sign a minor release form. You can download that here.

The Athletic Training department utilizes an online information and injury tracking software called Sportsware (SWOL). Each athlete is required to enter in their personal information and update it annually. To enter or update your information, follow the instructions below.
• From your browser window navigate to www.swol123.net
• Click on Join Sportsware on right side of page
• Enter School ID: TC Cougars
• Select “Next”
• Enter your first name, last name, e-mail and group (Taft College)
• Select “Send”
• Before you can continue you will need to be accepted. This may take up to a day to complete.
• Check your e-mail for acceptance notice. Sender will be “admin” and subject will be “SportsWare OnLine (SWOL) Password Request”
• Click on the link and enter and choose a password
• Enter in your information under all 5 tabs labeled General, Address, Emergency, Insurance and Medical.
• Click on the “Medical History” button above the tabs and complete the history questions. Any questions you answer “yes” to, please provide a brief explanation.
• Only enter information marked with a red asterisk *
• Select “Save” when finished. If you do not receive an error message you are done

Your insurance ID card and physical form are still required to be presented to the Athletic Department Secretary (Jennifer Edmaiston, 661-763-7829) for verification.

Concussion Baseline Testing Information
Taft College Athletic Training utilizes the SWAY system for baseline testing and post-injury evaluation. SWAY utilizes an app which will be downloaded to the student athlete’s phone. The student-athlete will
be given instructions distributed through email by the TC Athletic Trainer. There are normally several group sessions over the summer months (June-August). You must complete your SWOL information prior to SWAY testing. This must be completed and accepted prior to an athlete participating in any practice.
